This is the Dark Aero Theme having a top shellstyle.
(Updated the window alignment, thanks to Peter-o:[link])

The File Contains:

- The AeroDark theme and visual style (self extracting pack)
- The Styler Toolbar AeroDark theme
- ViOrb

Have you patched up uxthemepatcher?? If not do it because inorder to use 3rd party themes or visual styles you have to do it first, else they won't be enabled.

First extract the files to some folder. Inside that folder goto Themes folder, double click on the AeroDark.exe file, it will automatically extract the theme and visual style in C:\Windows\Resources\Themes. After that right click on the desktop and select themes...here you will find Aero Dark theme, select it and click apply.

For the styler theme, copy the AeroBlue folder present inside Styler folder to C:\Program Files\Styler\TB\skins\Styler's\ folder. Then select the styler theme...and its done.

Copy the Viorb folder to C:\Program Files. Open the folder and open the ViOrb2.exe file to run it.
